If you earn an achievement, and earn some later on in the same path, it will visually undo that a previous one in that path was earned. It happens so consistently that if you make about 8 mythic achievements, you WILL see this bug.
I am not going to provide server files. I promise you that you will easily recreate this bug from any config. There are no errors.
Noteworthy that I am using all latest dev builds of Mythic Mobs, Crucible, HUD, RPG, and Achievements.
