Could it be your older MC version? Has it worked in the past? Also, they are probably going to want to see the startup log/latest.log for your server.

I tested this with a 5.9.5 and your item seems to work fine on 1.21.1 closing as I can't reproduce, if you have extra info on how to reproduce, you can reopen the issue.
